Forces you to do everything on their app, which has no accessibility features. At the very least, if they're going to be in cahoots with insurance companies to force people to use their services or no longer cover medications, then make a more accessible option for people with disabilities.

An unessesary middleman in delivering healthcare

These new tech first companies do not have everything figured out yet, so be careful with your expectations. Sometimes they can fix a problem but do not have a way to communicate. So you need to enable them with a telephone! They do not see the need for telephones in modern business and that kept me waiting an extra week for meds! That's not care, that's negligence!

I absolutely love FORM HEALTH. I like how they have the app set up how you have the appointment set up and how easy it is to see the weight loss the encouragement of the dietitian and the doctors. I'm excited about my weight loss journey.

I began with Form Health in September. I was quickly assigned a clinician and dietitian. They were both present and helpful for all appointments. They were completely invested in my health and wellbeing. Early last month I was switched to a new clinician and he has been just as wonderful . The patient support is excellent and Staci Mullis responds quickly and gets me any assistance I need . Wonderful company !

I’m paying 4x what I was paying when I was seeing a doctor at one of the best hospitals in the country and talking to my doctor feels like talking to a teen nephew. It’s unethical for my insurance provider to force me to get meds through formhealth especially when I’ve been on them for 5 years and it cured my sleep apnea. I’m absolutely livid. No one who was forced to switch in my community is reporting coverage denial, so no one is saving money here. It’s all a scam and they should be ashamed.

Please realize this is not going be the same cost as your old coverage. Be prepared to pay your full deductible for prescriptions 1st and than 30% of that afterwards. On top of their monthly fees.

My doctors are very attentive and hands on, available whenever I need them. It’s so convenient to set up appts and get questions/concerns answered!! Love it!

Your team is excellent. Follow up and communication is good. My main constraint is access/availability. If I had a lot of spare time, I'd exercise more and probably not need the extra coaching... Not be fat. It would be extremely helpful to have night and weekend hours.

The weight loss journey has been a challenge for me throughout my life. The experience with the Dr and dietitian at Form health has made a tremendous difference in my current weight loss experience and success.

This service is great if you dont have to pay (ie. when it essentially is free). My insurance used to pay but now they have caught up and will not cover it. For me, it is hard to see that this is worth $149/month with an extra charge for any consultations.

The feedback and guidance has been specific and collaborative with both the doctor and dietitian. Honest and supportive communication about the program, process and what to expect. Great experience and the program is effective. I highly recommend this team.

I went through the whole process, filled out all the information and paid for this. I never heard from anyone. I was never assigned to a team and no one ever called or emailed. It was just a scam to steal money. It’s been over a month total scam. I had to fill out a form to cancel and they will review. That does not work for me. You will cancel and you will return my money because you didn’t do anything. You will return my money or we can do this in court, but I am so sick of Internet scams I can’t take it anymore.

Nobody ever answers calls, EVER! The virtual agent does not understand anything and they never call you back as promised.

It a been extremely difficult to reenroll with form health. It has taken several weeks.

UPDATE: 1/30/2025 - NO ONE HAS REACHED OUT TO ME AS STATED IN THIER RESPONSE HERE. DON'T LET THEM FOOL YOU. IT SEEMS TO BE THE WAY OF THE WORLD TODAY TO SPEAK UNTRUTHS. I WILL EDIT THIS IF I AM INDEED EVER CONTACTED.

If you are looking for a company that shows little concern for your health, this is the company for you.
After an incident with one of their providers, I spoke with " the lead of clinical and commercial success" . It quickly became clear that Form Health does not prioritize patient health or safety. Being prescribed medication after a 15-minute virtual visit by someone who does not know my medical history—compared to my primary care physician who has treated me for over 25 years—is incredibly unsafe.
When I experienced side effects from the medication, the Form Health provider offered no meaningful support. I was never properly informed of potential side effects beforehand, and when they did occur, I was not reassured that they were normal or temporary—something both my primary care doctor and the medication manufacturer confirmed. Instead, the provider escalated my anxiety by telling me to go to the emergency room and get an ultrasound. That was the full extent of the “care” I received.
This experience caused unnecessary stress and panic during what should have been a medically guided process. "The lead of clinical and commercial success" ultimately defended the provider and offered no compensation, despite the fact that I still have three out of four unused medications and the undo stress and anxiety that I was put under.
To make matters worse, my wife’s appointment was canceled by Form Health on the same day she was scheduled to be seen—yet if patients cancel the same day, they are charged an excessive fee.
I strongly recommend avoiding this company if you have any other option. Stick with providers who actually know your body, your history, and treat you like a human being—not a transaction. Form Health appears far more focused on profit than patient care.

The issue is they are not doing anything the PCP cannot do. They are forcing you to meet with three different doctors on the site as opposed to your one PCP to deter you more. It is easier to go through WW to get the meds I need. There is no true communication and it takes forever to get any help
